Free & CrossPlatform. I LOVE IT!
Mac too! Great little password generator included. Actually KeePassX is for Win, Mac & Linux but as a port of the original KeePass Windows users are probably better off using that - it's got plugins and stuff.
Linux counterpart for Keepass that does everything it claims to do.
